My child's participation in Flying Feet LLC's program selected is voluntary. I understand the selected activities may involve accidental injury, and I take responsibility for these risks. Knowing these risks, I want my child to participate in this activity. I (on behalf of my child) assume the risk and waive, release, and discharge Flying Feet LLC, its officers, employees, activity instructors, and assistants, and all officers and employees of the school or community center sites where the said activity will take place, for any and all claims for damages for personal injuries, or claims for damages to property, which my child or my child's heirs, assigns, executors or administrators may have or which may accrue to my child's participation in this activity.

I consent to Flying Feet, LLC to use any photographs or video recordings taken of my child while participating in the activity for use in Company brochures and program materials distributed as printed documents and on the internet. You will not receive payment for the use of these photographs and videos. Flying Feet, LLC will never use your child's name in connection with these images. *

REFUNDS:
Flying Feet, LLC will not issue refunds for cancellations or withdrawals.

COMMUNICATION:
I consent to Flying Feet, LLC to communicate via phone, email, and text during the service. You'll receive a text message from Sheila Haunert asking you to join a chat via GroupMe. You don't need to download the app to join the chat.

PUNCTUALITY POLICY:
Our coaches are extremely talented and dedicated to their work, and we kindly request parents to be respectful of their time by being punctual for drop-off and pick-up. We request you to please return to the field 15 minutes before the end of the run club class to ensure you are there when we finish. Our coaches have other commitments after the run club, and therefore, it is important to pick up your child on time. In case of late pick-up, Flying Feet Run Club may charge a late fee to parents who routinely fail to pick up within a five-minute window. If you know that you will be late picking up your child, please notify your coach ahead of time by calling or texting them.

WEATHER:
We are a rain-or-shine club. Flying Feet, LLC may use alternate practice facilities or schedule a make-up session if the weather does not allow field use. We'll add a maximum of two make-up days if we need to cancel.

BEHAVIOR:
Flying Feet, LLC reserves the right to release any child whose behavior becomes unmanageable without refunding program fees.

COVID-19 has been declared a worldwide pandemic by the World Health Association. BRC has put in place preventative measures to reduce the spread of COVID-19. I agree that myself and my children will not attend our running session if anyone in our household has been experiencing any of the following symptoms:
Temperature at or above 100.4 degrees Fahrenheit;
Persistent, dry cough;
Shortness of breath or difficulty breathing
~ Or at least two of these symptoms:
Chills;
Repeated shaking with chills;
Muscle pain o Headache;
Sore Throat;
New loss of taste or smell
